If you are out-of-network, I provide a receipt of services provided for you to submit to your insurance for reimbursement. I am happy to support you in this process.
I am in network with BlueCross BlueShield PPO plans. Your cost per session will depend on your individual deductible and copay. Prior to starting therapy, I will provide your family with a 'good faith' estimate.
For private pay, these are the following costs:
$200 - initial evaluation and comprehensive report.
$150 - each treatment session.
Pricing for consultative services are determined on a case-by-case basis.
Consistent attendance and family participation is key to effective therapy. If you absolutely need to cancel, please do so at least 24 hours ahead of time.
I understand life happens and emergencies are unpredictable! If that is the case, please explain the situation and I will do my best to reschedule your session later that same week.
For minor situations, short notice cancellations will be counted as no-shows. After a total of 3 no-shows, I will no longer provide OT services.
For cancellations within 24 hours of the scheduled session, $50 will be charged. For no-shows, the cancellation fee is the same as a regular session, $150.
I work with babies as young as 5 months old and children up to 12 years of age.
No diagnosis required for services. I can work with your family regardless of medical background.
